Yes, the information on your insurance card is needed for us to file a claim with your insurance company or companies. When you arrive at our office we will ask for information about your insurance coverage and have you sign a few forms. This registration process will go much faster when you bring your insurance information with you. Your driver’s license and or a picture ID is required as well.

This will depend on your health plan. Some plans require that you contact your PCP (primary care physician) for a referral. The referring doctor or PCP’s office will begin a referral request into their system. A nurse and/or physician may review your referral request and make a decision based on coverage and medical necessity issues specific to your plan. A determination may not be made immediately if all necessary information is not available, or if you call for the referral too close to your appointment with us. In many cases the PCP may not be able to issue the referral on the day of your visit. You need to call ahead, preferably at least 2 days in advance for a referral. To avoid delays we strongly recommend you call your PCP’s office the day before your visit with us to ensure that they have sent us the referral.

Yes, however, because you have chosen to bypass your insurance plan requirements for a referral you will assume financial responsibility for all costs associated with he visit.
Coverage varies with each insurance company and plan. Not all medically necessary and appropriate services are covered by your insurance contract. Please refer to your insurance member handbook or call your insurance company with questions. If you are unable to get a satisfactory answer form them please call our office, we will be glad to help.
Yes, as a courtesy we will bill your insurance company. It is your responsibility to provide any requested information to your insurance company (accident information, claim forms).

We agree with you, the amount of paperwork you have to complete is frustrating. Unfortunately, we are required to submit information to your insurance company and having you fill the forms is, at this time, the most efficient way to gather this data. We have to comply with their requirement to submit a claim for your services. The rest of the forms are medical questions designed to assist your doctor during the examination. We have done our very best in reducing the amount of paperwork we present to you. We sympathize with your frustration, after all we know what you are going through - we have to fill the same forms when we visit our doctor! In order to complete these forms we ask that you arrive at our office at least 30 minutes before your scheduled appointment, this will allow you ample time to complete the forms.
